Programs

Every program. One Scorecard.

Most agencies run more than one kind of program, and each ends up in its own tool. In Condor, every program feeds the same Behavior Scorecard and the same Outcomes.

Turn on only what you run

The problem

A long menu of programs you do not run confuses staff.

What Condor does

The program catalog lets an agency turn on only the lines it runs, and each unit is tied to its program.

  • Program catalog
  • Units tied to programs
  • Fourteen-day line for each program

Each program keeps its own work

The problem

Generic forms make a foster visit look like a cottage shift note.

What Condor does

Each program keeps the work of that line, and notes start with what went well.

  • Residential cottages: the unit, the shift and what went right, plus internal QRTP and 30-day review notes
  • Therapeutic foster homes: home study, certification date, training hours, placement match, respite and permanency
  • Day school: attendance, IEP goals and education notes, one period at a time
  • Community and prevention: visit notes and a case plan
  • Clinic: counseling contacts, kept apart from the med pass
  • Independent living: skills, aftercare and work hours on a checklist you write
  • Juvenile justice: an internal case and incident
  • 29-I health home: a care-management touch

Every program feeds the Scorecard

The problem

When each program lives in its own tool, nobody sees the whole person.

What Condor does

Wins from every program desk land on the Scorecard and roll into Outcomes and the grant packet. Each person’s program notes sit on their profile in time order.

  • Practice models: your own checklist and caseload limit
  • Enrichment: animal, farm, music, outdoors, service or coaching
  • Points and rewards: your agency names the currency, and they land on Wins
  • Specialty cases: immigration milestones, support goals, rapid re-housing, adult residence and restricted safety plans
  • Resident timeline

Honest limits

  • Condor does not file with a court.
  • A 29-I note is not a claim and not a state file.
  • The day school desk is not a student information system.
  • Lists named for 837, 29-I claims, CONNECTIONS, court e-filing, NSP/LSP packets, Fair Futures and QRTP certificates are internal lists, not those official files.
  • Condor does not certify a practice model and does not include a licensed manual.
  • Restricted safety plans for trafficking and harmful behavior are limited to clinicians and directors.

It all comes back to the Scorecard

Every log, note, program and report in Condor feeds the Behavior Scorecard, and the Scorecard feeds Outcomes. One record, one picture of how each person is doing.
